automatically remove ?referrer=raid from twitch urls

v2.0.0 17/02/2021 performance & stability improvements: 1) now works with urls that contain other parameters (such as `twitch.tv/example?foo=bar&referrer=raid&baz=qux`) , and 2) hook `window.history.pushState` rather than constantly checking the url on an interval, which will improve performance and make the script faster to activate when a raid happens
